Care ManagementWe provide clients with Community Health Workers whose main focus is linking and re-linking clients to primary and specialty care as well as connecting persons to supportive services.
Application Assistance with SNAP (food stamps), CHIP, TANF, Social Security, behavior health referrals, and navigation through other services that are offered throughout the community such as the Affordable Care Act Insurance and Medicaid. Our CHWs have a wealth of knowledge for Information and Referrals. When a client's basic needs are being met, overall health outcomes are greater. Prevention Services PrEP (Pre-Exposure Prophylaxis) is the newest tool in HIV prevention. It's a prescription medicine that, when taken every day, significantly reduces your chances of getting HIV by blocking the virus. PrEP does not protect against sexually transmitted infections like gonorrhea or syphilis, so you should continue using condoms to prevent the spread of sexually transmitted infections. Whether you have insurance or not our Prevention Specialist can link you to the appropriate financial assistance program/s based on your individual circumstances. In most cases, you are able to access the medication at no cost to you. |
HIV Positive Specific Services
Health Insurance Premium and Co-Pay Assistance Program This program is provided to assist clients, who may qualify, in paying for Health Insurance Out-of-Pocket Costs such as Premiums and Co-Pay. Qualifications: Must have insurance, Income guidelines do apply Housing Program and Emergency Financial Assistance This program is provided through federal funding to assist clients, who may qualify, in paying for long-term and short-term rental/mortgage, and utility assistance. Qualifications: Recent financial emergency (Lost job, laid off, hours cut, recent discharge from hospital etc.) |
Food PantryEast Texas Cares Resource Center has been serving the East Texas area for over 30 years. Our pantry remains to be a source of not only food but empowerment and hope amid despair for all those we serve. At our Pantry, it is our job to ensure every person who walks through our doors is treated with the topmost respect as they receive healthy meals, fresh meats, and fresh produce to prepare for their families.
Open Mon.-Thurs. from 11:00am – 2:00pm. They are supervised by a licensed psychologist. Services offered: • Individual therapy • Family counseling and psychoeducation • Group therapy • Brief neuropsychological screeners • Psychological evaluations |
